Discharge & Aftercare

Quality care after your hospital stay – for lasting treatment success

Our doctors, nursing staff, and other employees do their utmost to ensure that your inpatient stay at Animal Hospital helps you get better soon. Before you are discharged, a final consultation with your attending physician takes place. You will receive your discharge summary and, if applicable, a medication plan. Additionally, if necessary, the doctor can issue prescriptions and a certificate of incapacity for work to bridge the gap until your next appointment. You should anticipate — especially after a prolonged hospital stay — that you will require ongoing medical treatment and potentially further care or follow-up support. Therefore, with your consent, we send the final discharge summary directly to the practice that will be providing your continuing care.

 

Our goal is to ensure that, following your discharge, you receive continued care tailored to your needs — whether medical, nursing, or social. This requires the structured and secure transfer of information essential to your care — for example, to the doctors providing follow-up treatment or to health and long-term care insurance providers. If you require continued nursing care or household assistance after leaving the hospital, the doctor will prescribe these services. Our social services team ensures that these prescriptions are implemented.
